D3 Youth Conference (grades 6 to 12th) is a two-day event hosted by Boyce College, crafted for Christian high school students, centered on answering a central question of Christianity. Child Transportation Release

STATEMENT OF PERMISSION AND RELEASE:

I, the undersigned, hereby grant the above-named student permission to travel with a staff member or volunteer in relation to Hillcrest Baptist Church, and to participate in services of Hillcrest Baptist Church. I, individually and on behalf of my child, hereby release, indemnify, and hold harmless, to the fullest extent permitted by law, the person named above and Hillcrest Baptist Church and its trustees, officers, employees, volunteers, members, and agents, from any and all actions and claims for personal injury, gross negligence, willful or wanton conduct, or damages of any kind (known and unknown), resulting or arising from, or relating to, my child’s transportation in relation to or participation in church events, services, and functions, including transportation in vehicles whether or not owned or leased by Hillcrest Baptist Church, whether caused in whole or in part by the actions or omissions of Hillcrest Baptist Church or its trustees, officers, employees, volunteers, members, or agents. I understand that Hillcrest Baptist Church will not be supervising or monitoring my child’s transportation.

Bathroom Assistance Permission

At Hillcrest Baptist Church, we strive to protect and help your child as best we can. All HBC volunteers have received training and background checks. Should your child need assistance in the bathroom due to vomiting, buttoning clothes, clean up, etc., we ask that the parent/guardian grant us permission to assist your child.
